Complete this form to apply for Traffic Clearance Certificate, usually required by those applying for renewal of taxi and public transport licenses. Once submitted, your application will be reviewed and you should be notified within 5 working days that your application has been received. At that time you will also be informed when to collect the report.
The fee is $25CI (or $75 for an express application, which will be processed within two days) for a certificate. Kindly upload a copy of your driver's license AND your passport with your application. Even if uploaded, your driver’s license and passport are required to be presented when you pick up the certificate.

Those picking up a record on behalf of someone else must have a signed and notarized letter from that individual authorizing the release of the record to you.
